Overview

Job Role Summary

The Advanced Practice Provider (APP) – Hematology Oncology will plan, direct, and evaluate patient health care for Eskenazi Health patients, under the direction of a collaborating physician. This individual will provide the full scope of services under their individual field of training, including but not limited to diagnosis, treatment, counseling, and coordination of care

Responsibilities

Essential Functions and Responsibilities

• Collect detailed health history, diagnose and treat a diverse patient population with empathy and formulates assessment and treatment plans to the best of their knowledge, specifically related to hematology/oncology conditions.
• Order and interpret key diagnostics (i.e. CBC with differential, CMP, coagulation studies, basic smear concepts, relevant imaging basics) to incorporate into the patient’s plan of care.
• Works closely with Attending Physician, Fellow, or Senior APP on complex patient issues and any time that the APP requires assistance.
• Refer patient to appropriate follow up appointments and transitions of care.
• Strive to exceed patient expectations, enhance the patient experience following Eskenazi Health AIDET.
• Consistently seek excellence in always living out the PRIDE and values and in all interactions with patients, families, staff, providers and others.
• Complete all medical record documentation and billing submissions within expected timeframes.
• Engage in meetings and adhere to Eskenazi Health and EMG bylaws, policies, and procedures.
• Stay at the forefront of medical advancements by integrating evidence-based practices into your care and contribute to updating practice protocols and principles.
• Facilitate collaboration, communication, and coordination with physicians, healthcare personnel, and care team members.
• Maintain appropriate and up to date licensing, certifications, and continuing education.
• Provide feedback, when appropriate, regarding clinic staff performance and clinical competencies to clinic staff and management as needed.
• Contribute to Eskenazi Health and EMG missions by participating in quality improvement initiatives, coordination of care and/or optional teaching opportunities.

Qualifications

Minimum Qualifications, Training, and Experience

  •  Successful completion of an approved APRN or Physician Assistant program and clinical requirements.
  • Active licensure as an APRN or PA in the State of Indiana by the Medical
  •  Licensing Board, prior to hire date.
  • Insurability (malpractice)
  • Current DEA, CSR Registration
  • Current BLS or PALS
  • Must be able to meet and maintain current Eskenazi Health credentialing and privileging requirements as applicable.

Working Environment

  • Outpatient clinical setting.
  • High contact with team members and must come into close physical contact with patients during visits.
  • Must wear required clinic attire and appropriate PPE.
  • Capacity to sit/stand for extended periods.
  • Mobility to move about hospital and patient rooms readily.
  • Able to lift 10 pounds for routine performance of essential functions.
  • Manual dexterity to appropriately use medical equipment.
  • Perform repetitive hand motion and good body posture due to extended use of computer system for patient documentation and other purposes.
  • May be exposed to infection, disease, adverse indoor and outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
