Overview

The Advanced Practice Provider – Inpatient Palliative Care works as an educator, advocate and care provider and is responsible for the care and well-being of patients followed in the Palliative Care program.

Responsibilities

• Independently carry varying inpatient caseload depending on patient census and acuity
o Provide consultation in palliative care, symptoms management, and supportive care to meet the needs of the patient as requested by the referring provider.
o Facilitates clarification of patient and family goals of care.
o Facilitates family/interdisciplinary team meetings.
o Facilitates access to appropriate support models of care (i.e., hospice, rehabilitation, on- going aggressive treatment.)
o Assists with diagnosis, treatment, and management of acute and chronic health conditions.
o Consults with palliative care physician as needed, informs primary provider of recommended services, and collaborates with other providers as needed.
o Prepares and maintains accurate patient records, charts, and documents to support the medical practice and reimbursement of services provided.
o Prescribes medications including controlled substances within the scope of practice and collaboration with palliative care and referring physician.
o Orders diagnostics/treatments as needed after collaboration with palliative care team and preferring providers.
• Will independently, or in coordination, carry a varying caseload of patients in the outpatient/ambulatory setting. May include patients’ home, nursing home, oncology clinic, or within an individual palliative care clinic.
• Assists with completion and documentation of advanced care planning with patients.
• Assist with planning appropriate patient follow up with interdisciplinary team.
• Manage the patient list for the inpatient team daily. Disseminate the patient list to Palliative Care staff (physicians, social workers, nurses, etc.), spiritual advisor and learners.
• Facilitate daily interdisciplinary team huddle and ensure timely completion.
• Primary team contact for phone calls and pages. Responsible for consult requests and triaging consults and assigning them to providers and learners.
• On-call coverage for eight weekends a year. (Friday 5pm to Monday 8am and one day of rounding.)
• Completes hospice referrals and collaborates with social work to complete the referral.
• Assist with education;
o Healthcare providers regarding advanced care planning
o Regarding hospice to patients, families, and health care team members.
o Assist team with education presentations to other departments and interdisciplinary teams as requested.
o Provide “board talks” regarding various palliative care topics.
 Precept nursing students.
 Facility staff, coworkers, and community on all aspects of palliative care and services provided by the Eskenazi Health Palliative Care Program.
• Assist with facilitation of learner education by assisting with orientation to rotation, didactic information, fielding questions, and daily education to medical students, residents, and fellows.
• Assists in the development of clinical practice guidelines/standards in support of quality palliative care as requested
• Participate in group quality improvement plan
• Attend monthly team meetings, biweekly/monthly provider meeting, and fellow education sessions.

• Exercise judgment/make decisions to determine extent of problems and to decide on appropriate course of action.
• Possess excellent interpersonal skills and can work effectively with a diversity of personalities. Must be approachable and present date with effective communication and presentation skills.

Qualifications

• Expertise in the practice of nursing validated by experience in current clinical practice including experience in the area of specialty practice.
• Specialty certification if available/appropriate.
• Hold prescriptive authority according to Indiana statue and institutional guidelines.
• Hold or obtain current Indiana Controlled Substance and Drug Enforcement Registration.
• Acute Care and/or Critical Care experience a plus.
